A polio vaccination certificate is a health document that may be requested for international travel or as part of vaccination history. Immigrant visa applicants also have separate CDC vaccination requirements administered during the panel-physician exam.
Why it may be required
U.S. immigration law requires certain vaccinations before an immigrant visa is issued; CDC publishes the immigrant vaccination list, which can change. Pakistan and transit countries may also impose exit/entry polio documentation that is separate from the U.S. visa medical. Because health and travel rules change, verify the current requirement before travel.
Who normally needs it
Anyone to whom a current CDC immigrant-vaccination rule or a current Pakistani/WHO travel requirement applies. This site does not state that every applicant always needs a specific paper polio certificate from a named clinic.
Where Pakistani applicants can obtain it
Vaccination records may come from prior immunization providers. Required immigrant-visa vaccinations are assessed by embassy-approved panel physicians. For Pakistan travel-health certificates, confirm the current issuer with official Government of Pakistan health authorities.
