Pakistani passports are issued by the Ministry of Interior, Directorate General of Immigration and Passports. Ordinary passports have a green cover. The Department of State states Pakistani passports are valid for five or ten years.
Why it may be required
The Islamabad supplement requires an unexpired passport valid for six months after the date you plan to enter the United States, plus a photocopy of the biographic page.

Confirm identity, photographs, and supporting documents on the official passport website. The Department of State notes Pakistani names may appear in varied combinations and that NADRA processes are used for legal name changes.
