Death certificates in Pakistan are issued by Union Councils, Cantonment Boards, Municipal Corporations, and the CDA for Islamabad, according to the Department of State.
Why it may be required
Needed when a prior spouse is deceased, or when other civil-status facts depend on a death record. The embassy checklist lists a spouse’s death certificate as an alternative to a divorce certificate for previously married applicants.
Who normally needs it
Applicants whose case depends on proving a death, including some widow/widower and prior-marriage cases.

The applicable local government authority, based on proof of death as described on the reciprocity page.
General application process
The Department of State states a hospital report of death is acceptable proof in some cases; deaths at home may require affidavits from close relatives or neighbors; cemetery receipts may be accepted where a cemetery registration system exists.
